Description:

Socrates 360 is an app and webpage that has been designed to help children take control of their health and wellbeing.

The app is offered to the children at the pre-discharge meeting.  If they accept, the healthcare administration team set up a user account and provide a postcard containing their login details and download instructions.

The child’s health summary is downloaded to the app, giving them instant access to their own personal health passport, allowing them to share key health and wellbeing information with any agency involved with the young person’s health and wellbeing in the community.

There is also a diary feature which allows children to keep track of any upcoming appointments and deadlines. The children can “check in” via the app to show that they have attended their appointment and record relevant details.

In addition, the app provides access to local information, and key services including GPs, Pharmacies, Accident and Emergency, Walk in Centres, Health Clinics, mental health facilities, Citizens Advice Bureau Hostels, Night Shelters, Food Banks and a useful map facility.   

Also included is a section on health advice to help develop individual awareness of health and wellbeing.

This app is only currently available for children that have attended Werrington Youth Offenders Institute (YOI) and Feltham Youth Offender Institute. All initial enquiries should be referred to Sarah Hayes.
