Description:

Oldham Youth Justice Service has developed a form to complete before a child applies for post-16 educational provision. The service shares these details with the team’s educational provision safeguarding lead. The form discloses the child’s level of need, safety and support. It includes sections on the crime they committed, any relevant risks or vulnerabilities and the support available to them.

The form makes up part of the below process:

  • The service meets with the child to identify the approach to their education provision
  • The service secures both the child’s and their parents’ or carers’ consent to approach a training provider and share their information
  • Complete the form and gain signatures for consent to share details
  • Contact the safeguarding lead at the relevant education provider to discuss the provision child is interested in applying for, and how best to meet their needs
  • Once the provider has stated the level of need can be met, the child can apply for the education provision.

This form has supported a number of children in Oldham to secure education, training and employment opportunities, helping to reduce their likelihood of reoffending.
